At Forget Me Not Children’s Hospice, we do things differently. Our pioneering and nurse-led service is built around the children and families we support, and is rated ‘outstanding’ by the CQC.
As Care Co-ordinator, you’ll lead a multi-disciplinary team providing holistic palliative care including planned and emergency respite, symptom control, end of life care and bereavement support for a defined caseload of children and families. You’ll co-ordinate care in families’ own homes as well as in the hospice, delivering hands-on care as well as supporting other members of the care team to deliver the high quality, family-centred service we’re so proud of.
This is an extremely rewarding role, not least because, unlike so many other healthcare environments, we are in the privileged position of having the time to deliver holistic care to children and families, throughout the palliative care journey. Not only that but we provide comprehensive induction and ongoing training, as well as resilience support and one-to-one supervision.
