Chestnut Tree House is the children’s hospice for East and West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-limited conditions.
This is an exciting role for an experienced nurse to provide nursing care and leadership in the community setting. The post holder will predominantly work in the community, managing a small team of nurses and care support workers, managing a small caseload and providing skilled palliative and end of life nursing care including, on-call provision to CYP accessing CTH services. The post holder will have responsibility for key working, supporting both children and families living with a life-limiting condition. This role will suit a dynamic individual who will be able to deliver respite and end of life care in the home, take CYP on fun outings and access daycare at the hospice.
The post holder will have a key responsibility in networking and developing collaborative relationships across health, social care and other voluntary services.
This role will also have responsibility for leading on transition within the children’s service and will be required to work with stakeholders to establish clear transition pathways, support networks and outreach for young adults. The post holder will develop a hub of specialist information for children and young people and will drive collaboration across the system as well as providing advice, support and education to the clinical team to support all young people transitioning through palliative care services.
The post holder will be supported with a comprehensive clinical skills training programme and will also be developed and supported to provide end of life care in the home setting.
This role is suited to a dynamic and innovative individual who is committed to service improvement and supporting young adults in transition. The post is predominantly based at the Eastbourne office but will require occasional travel to the CTH hospice in West Sussex
