Chestnut Tree House is the children’s hospice for East and West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-shortening conditions.

This is an exciting role for an experienced nurse to provide nursing care in the community setting. The post holder will predominantly work in the community, managing a caseload, and providing skilled palliative nursing care to all Children and Young People accessing Chestnut Tree House services.

This role will suit a dynamic individual who will be able to deliver respite care within the home and take the child or young person on fun outings and access daycare at the hospice.

The post holder will have responsibility for key working, supporting both children living with life-limiting conditions and their families. They will be supported with a comprehensive clinical skills training programme and will also be developed and supported to provide end of life care in the home setting.

This role is suited to a dynamic and innovative individual who is committed to service improvement and supporting young adults in transition.
