Chestnut Tree House is the children’s hospice for East and West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-limited conditions.

This is an exciting role for an experienced nurse to provide nursing care and leadership in the community setting and to lead on end-of-life care in the locality. The post holder predominantly works in the community, managing a small team of nurses and care support workers, managing a small caseload, and providing skilled palliative and end of life nursing care including, on-call provision to CYP accessing CTH services.

In this position, you have responsibility for key working, supporting both children and families living with a life-limiting condition. This role suits a dynamic individual who will be able to deliver respite and end of life care in the home, take CYP on fun outings and access day care at the hospice.

The post holder has a key responsibility in networking and developing collaborative relationships across health, social care and other voluntary services.

This role also has responsibility for leading on end-of-life care within the East Sussex locality and are required to work with stakeholders to establish clear end-of-life pathways and support networks. You will develop a hub of specialist information for children and young people and will drive collaboration across the system as well as providing advice, support and education to the clinical team to support children and young people using palliative care services.

The post holder will be supported with a comprehensive clinical skills training programme.

This role suits to a dynamic and innovative individual who is committed to service improvement in palliative care, due to the locality the post holder will need to work autonomously. The role is predominantly based at the Eastbourne office but will require occasional travel to the CTH hospice in West Sussex.
