Rated outstanding by the CQC, at Forget Me Not we take a pioneering nurse and therapy led approach, providing care and support to over 600 families in West Yorkshire.

As well as providing clinical, palliative and end of life care, the support we provide to our families through specialist counselling, therapy and bereavement support is absolutely vital and has a major impact on their wellbeing, in the short and longer term.

Making your role hugely important. Working with families at times of acute distress, you and your team will provide psychosocial support, advocacy and advice, making a huge difference to how they feel in the moment, and in helping them cope with family life, education and employment in the future.

It’s a varied role – you’ll have your own caseload and devise care plans using your counselling skills and knowledge of bereavement and loss to deliver therapeutic interventions. You’ll help individual family members adjust to new situations and empower them to make their own choices and decisions.

You’ll support your team’s personal development, set up and manage governance processes, support individual team members in their roles, have oversight of the team’s caseload and maintain effective communication and liaison between Forget Me Not and other professionals in the community.

Ensuring best interests of the family are always at the forefront of everything we do, you’ll make sure high standards of family support are maintained and that staff follow correct governance and compliance procedures.

This is a fantastic opportunity to redefine and reshape this incredibly valuable service. So if you have bags of experience of working with children and families in a health or social care therapeutic environment, providing practical, emotional and advocacy support to vulnerable clients, as well as experience leading and supervising a multidisciplinary team.
