Hospice at Home Coordinator
RN(C) or RGN with experience
30-37.5 hours per week
This is an exciting time for Andy’s as we launch our new Hospice at Home Service. The service will deliver home based short breaks for vulnerable children, young people and their families from Hull, East Yorkshire and Lincolnshire. In addition, it will deliver a programme of activities based at the hospice and elsewhere in the region to promote wellbeing and maximise family support.
We are looking for a committed, passionate and creative individual to set up and drive this service, promoting the highest standards of patient care. RN (C) preferred but applications considered from RGNs with significant relevant experience. Palliative care and community experience preferred.
You must be able to work as an autonomous practitioner as well as part of a team. A commitment to a multidisciplinary approach is essential, alongside the ability to be self-reflective and maintain strong professional boundaries. The role includes elements of line management and a contribution to the on call rota. Flexibility is required, as although the hours are predominantly Monday-Friday office hours, some evening and weekend work will be required according to the needs of the service. This role has a significant element of community working therefore access to a vehicle is essential, as is a full driving licence.
