Together for Short Lives is the UK charity to make sure the 99,000 seriously ill children and their families can make the most of every moment they have together – whether that’s for years, months or only hours. A fantastic opportunity has arisen to join our Together for Short Lives team as a Kentown Family Service Coordinator. Together for Short Lives have been funded by The Kentown Wizard Foundation to deliver an exciting project, in partnership with Rainbow Trust Children’s Charity, to ensure every family caring for a child with a life-limiting condition can get the right care and support, based around the unique needs of the family at home, so families can make the most of every precious moment together.

The Kentown Children’s Palliative Care Programme is a community focussed model with three key complementary elements: Nursing Care, Social Care and Information and Awareness. The programme will invest in building on established children’s palliative care and support services, strengthening co-ordination and avoiding duplication to make life easier for families.

Your role as a Kentown Family Service Coordinator is to reach out into your community, linking families with children’s palliative care and other services in the region as well as with the national services provided through the Together for Short Lives Family Support Hub and Rainbow Trust Children’s Charity. The role will require you to develop strong links with professionals and services working in the region; and to support families to navigate the health and social care systems and to access the services they need for their child and wider family. You will be part of a Kentown team of specialist palliative care nurses and Rainbow family support workers.

We are seeking a compassionate, enthusiastic and inspiring individual who has the ability to communicate and engage confidently and professionally with families, colleagues and professionals. You will be committed to collaboration and partnership working, demonstrating excellent people and engagement skills to forge new relationships with a range of organisations and services. Working for a national charity in a new and unique role, you will have the opportunity to make a positive difference in children’s palliative care and help to improve the quality of lives for children with palliative care needs, and their families.

This role will be a home based 3 year fixed term contract. There will be travel within the region and to a satellite office in the region. The post holder must hold a full clean driver’s license.
