Then this may be the role for you, we at Rennie Grove have a rare opportunity to work flexibly across all services within Hertfordshire including Hospice at Home and Living Well Services providing specialist palliative care nursing to patients and families in their own home or Living Well Services.
You will be visiting individuals in their own homes so a car and driving licence will be essential.
You’ll need to demonstrate your abilities in the following areas:
First level registered General Nurse desirably with a module in palliative care or cancer care.
Post registration experience.
Proven experience of effective team working.
Experience of MDT working.
Ability to perform holistic assessment, plan and evaluate an individualised care plan.
Knowledge of medicines management appropriate to role.
Caring and empathetic approach.
