Community Registered Nurse

Chestnut Tree House is the children’s hospice for East, West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-limited conditions.

This is an exciting role for an experienced nurse to provide nursing care in the community setting.

The post holder will predominantly work in the community, providing leadership to healthcare assistants, managing a caseload and providing skilled palliative nursing care to all CYP accessing CTH services.

There will be opportunities to work across children services and support inhouse care. The post holder will have responsibility for key working, supporting both children and families living with a life limiting condition.

This role will suit a dynamic individual who will be able to deliver respite care in the home, take CYP on fun outings and access day care at the hospice.
