Nursing Associate for Hospice at Home

We are currently looking for a Nursing Associate for Hospice at Home Band 4 to join our Team here at Rainbows.

This is a full time position 37.5 hours per week and covers a 24/7 shift pattern.

In this exciting new role, you will work with a supportive team of Nurses and clinical specialists in visiting patient and family duties to support the efficient running of the service.

The hospice at home service is working as part of Rainbows hospice for babies, children and young people. Hospice at Home will be supporting end of life and palliative care for babies, children and young people within their own home, allowing them to experience the amazing Rainbows experience on their doorstep, “Rainbows wherever you are”.

You will be required to work as part of a pair with the Clinical Nurse Specialist, visiting patients in their own homes to carry out supportive patient care including documentation on SystmOne alongside other administrative and patient and family duties to support the efficient running of the service.

To be considered you will need to be a highly motivated individual. Have experience of providing palliative care and end of life care within the community setting.
