Transition Lead

We are looking to recruit to this exciting new role in our developing service. You will support the transition of young people with life-limiting or life-threatening conditions who access hospice care and are transitioning from Children’s to Adult Services.

You will lead on the transition pathway to effectively deliver the strategic aims of the organisation and act as a resource to colleagues regarding adult transition issues.

You will promote the needs of life-limited and life-threatened young people undergoing their transition to adulthood through collaborative working and partnership with other service providers.

If you hold a relevant professional qualification in either health care or social care, we would love to hear from you.
