We are looking for an enthusiastic and highly motivated individual to support the implementation and development of a new Transition Framework. Do you want to be instrumental in shaping and taking forward support for young people, their families and carers in transition?
You will have significant experience of working with adolescents with complex needs, demonstrating a clear understanding of the changes they face and the impact of transitioning out of children’s hospice care. You will have experience of working in a health care, social care or education setting.
To be successful in this role you will possess exceptional communication and interpersonal skills to enable you to build positive working relationships, as you will act as an advisor and an influencer to a number of teams across Demelza and external agencies. As part of the Family Support Department, you will receive regular supervision and support to manage this project and influence Transition within Demelza’s next five-year strategy.
You will be able to demonstrate excellent leadership skills and the ability to manage and engage staff. Exceptional verbal and written skills, a high level of organisation and the ability to manage and re-prioritise a changing workload is essential to this role.
