You will therefore need to demonstrate your abilities in the following areas:

  • Demonstrate and maintain consistently high standards of individualised personal care and support under guidance of the Rennie Grove Peace Hospice at Home management team.
  • Work alongside the patients’ families and / or health care professionals, maintaining an approach that incorporates physical, psychological, social, and spiritual aspects of care in accordance
  • with the Rennie Grove Peace philosophy.
  • Observe and report changes in the patient’s symptoms, conditions, and circumstances to the registered health professional, and notify other healthcare professionals as necessary.
  • Work in accordance with the patient’s agreed care plan.
  • Support the patient with any physical / personal care, if the situation requires, maintaining dignity whilst doing. This may include a simple dressing or carrying out last offices if required.
  • Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with Rennie Grove Peace policies.
  • Keep accurate and legible records in accordance with legislative requirements and Rennie Grove Peace practice and procedure.
  • Prompt medication if dispensed in a pharmacy blister pack.
  • Ensure all verbal and written communication is accurate and timely.

If successful, you’ll join on the first working Monday of the month so we can best support you with our structured induction programme.

Your main duties and responsibilities will be:

– Competently working alone, or with a registered health professional (nurse or paramedic), as required, with patients and their families in the home environment with support and guidance from qualified professionals.
– Respond to all calls and requests for patient visits, speaking with patients and relatives as required.
– To adhere to the lone worker policy and procedure.
– Working collaboratively with hospice and community partners.
– Acting as a Rennie Grove Peace ambassador within the community.
