As well as providing a rare insight into the views and experiences of young adults themselves; the resources reveal how small changes in practice, that do not cost anything, can make a real difference.
Together we hope these publications will be a useful resource for professionals working with young adult patients with significant, and potentially life-limiting, conditions and those with responsibility for supporting the transition from paediatric to adult care.
